The Kid Center Registration

Registration can be completed in 3 easy steps. You can print the registration forms now and bring them in with you or fill out the information on your first visit.

1. Download and Print Forms
You will need to fill out the Registration Packet to register with The Kid Center. To get started, simply download the files below and print them out.

2. Fill out forms accurately.
Remember to print neatly and fill in all information, including initials and signatures.

3. Bring them in!

Once you have completed these forms, you may:

When you visit The Kid Center for the first time, this information will be reviewed by our Director or Assistant Director. After all documents are complete, your family registration will be activated.
